Tyesha "Ty" Davis, referred to as "Coach Ty" and owner of Success Steps Coaching, LLC does not purport or make claim to be a healthcare or mental health professional. Coach Ty does not provide medical, mental, or healthcare advice, information, diagnosis or prognosis. Coach Ty is not a doctor, nurse, psychologist, psychiatrist, social worker, or any other designated mental health practitioner. Success Steps Coaching, LLC is not a medical practice, healthcare facility, or mental health program. Success Steps Coaching, LLC is a non-medical, professional coaching business that does not make any guarantees of client outcomes and success based on services received from Coach Ty and Success Steps Coaching, LLC.
The information, services, and recommendations provided through the coaching sessions at Success Steps Coaching, LLC are based on the work experience and educational experience of Coach Ty, Certified Life Coach.
If at anytime Coach Ty determines the client's needs are exceeding her realm of knowledge, expertise, and business offerings, Coach Ty has the right to terminate the coaching agreement, contract, session, and client relationship.
Additionally, Tyesha "Ty" Davis, "Coach Ty" has the right to refuse service if it is deemed a conflict of interest, inappropriate communication or behavior by the client or "potential client."
By booking a service through Success Steps Coaching, LLC, the user, client acknowledges and accepts that their (your) success is created and determined by you and that coaching is a tool to aid in awareness and understanding.
Confidentiality
All information obtained during session is confidential unless there is compelling professional reason for disclosure. In all circumstances, I will be judicious in the amount of information that is disclosed. When possible, I will inform you about the disclosure of confidential information and possible ramifications before the disclosure is made. I may disclose confidential information without your consent as required by law or if the confidential information may put you or someone else at risk of harm. I will only disclose confidential information to third parties with appropriate written consent.
Availability
Success Steps Coaching, LLC is available by appointment only.
Rates
Rates for each service and coaching program are provided in the coaching contract. It is the responsibility of the client to review the coaching contract for rates, terms, and conditions. The client's payment of invoice is acknowledgement of receipt and understanding of payment rates, terms and conditions. Rates are subject to change at any time. All coaching sessions must be paid in full prior to the coaching sessions.
In the event the client is paying in installment payments, the first half of the total amount of the coaching session must be paid prior to starting the coaching program. The remaining and 2nd installment payment must be paid prior to the start of the "Execution and Planning" steps of the coaching program.
If the 2nd installment payment is not received prior to starting the "Execution and Planning" step of the Success Steps Program, the client will have a 60 day grace period from the date of the original coaching session for the 'Execution and Planning" step to make the 2nd and final installment payment. If the payment is not received at the end of the 60 day grace period, the coaching program will stop and no further sessions will be conducted. Additionally, no refunds will be granted for services already paid for and rendered.
